Full Arch implants: 11 stages of treatment

Learn about the stages of treatment for full arch implant rehabilitation. A full dental implant is a treatment designed to rebuild a new smile when all the teeth in an arch or mouth have been lost. This type of treatment is also popularly referred to as a protocol prosthesis or All on four dental implants, the latter if it involves 4 implants.

Most full arch implant cases follow the following treatment steps:

1 – An individual assessment of the case for full arch/full mouth implantation is carried out

Considering the patient’s general and oral health, the volume and quality of bone in the mandible/maxilla, aesthetics, gum shape, among other aspects. To assess bone volume, radiological examinations such as panoramic radiography and computed tomography will be requested.

This stage also verifies the need for bone grafting prior to implant surgery, as well as the need for medical supervision if the patient has pre-existing diseases that could interfere with the success of dental implant rehabilitation.

2 – Treatment proposal

Depending on the information collected and the patient’s expectations, we will present some alternatives for full arch implant treatment with different stages of treatment, techniques and possible materials. The recommended types of dental implant will be presented according to the patient’s case, and they will be able to choose according to their preference. Today there are very modern dental implants and healing times can take between 1 and 6 months, depending on the type. Implants from the Swiss brand Straumann are the ones with the shortest healing times.

3 – Preparing the mouth

With the aim of eliminating sources of infection, such as professional cleaning, treatment of cavities, tooth extractions and periodontal scraping.

5 – Installation of full arch implants

Through simple and painless surgery, the implants that will support the fixed prosthesis are inserted. The surgery takes place in a doctor’s office and the patient can go home immediately afterwards. There are modalities with local anesthesia, or with intravenous sedation, if the patient prefers.

There is also the option of performing dental implant surgery using the computerized guided surgery technique.

This technique eliminates the need to make large cuts and sutures in the gums, making the post-operative period easier and the recovery quicker.

The surgical guide acts as a template with indications of where the incisions should be made to place the dental implants. The surgical guide is custom-made according to the patient’s CT scan images, using a 3D printing method.

6 – Installation of the temporary prosthesis

On the same day or after a few days. This is one of the most important stages of treatment. The prosthesis can be fixed or removable, depending on the case. The preference is for a fixed temporary prosthesis, unless this is technically impossible.

This first provisional prosthesis for total implants does not always meet all the requirements of aesthetics and function because it is made under adverse conditions, such as pre-existing teeth that make molding difficult. If necessary, we will make new prostheses.

7 – Waiting period

It takes between one and six months for the implants to integrate into the bone (osseointegration) and for the gums to heal.

8 – Start of the definitive prosthesis for total implants

If the implants are hidden under the gums, the first step is reopening surgery. This is a simple procedure that aims to expose the implants in the mouth. Then molds are made.

9 – Studies for the definitive prosthesis

For total implants, the aim is to find out which characteristics will be important in order to obtain the best aesthetics and functionality. When the characteristics of the final prosthesis are discussed, the color of the teeth, the shape of the teeth, the volume of the prosthesis and whether or not artificial gums are needed will be defined. The patient takes part in this process and will be able to see the stages of the treatment beforehand, as well as the final aesthetics. The dental implant prosthesis can be made of Zirconia, Porcelain or Resin, each with different characteristics, and the patient’s choice is made right at the start of treatment, so that planning is already carried out in anticipation of the definitive prosthesis.

10 – Delivery of the total implant prosthesis

With the approval of the patient and the dentist, the protocol prosthesis is then given as completed and all stages of the proposed treatment have been fulfilled.

11 – Total implant control and maintenance

Once all the treatment stages have been completed and the protocol prosthesis has been delivered, the patient should return every six months for a check-up and cleaning. Every twelve months we will request a panoramic radiograph to monitor the case internally. Even though the patient has had implants and, in many cases, has had all their natural teeth removed, they should still visit the dentist regularly every 6 months.

Implanting several teeth at the same time

The loss of multiple teeth can certainly affect your ability to chew, as well as the beauty of your smile. Implants with 3 teeth, 4 teeth or more have some advantages over other types of treatment with removable prostheses. As an alternative to placing several implants, we only have techniques that use common removable prostheses, without the use of implants. However, the indication is made after evaluating each case.

Should I have several teeth implanted or wear dentures?

Multi-tooth dental implants are an alternative to conventional dentures for those who have lost many or all of their teeth. They consist of placing metal pins, usually titanium, in the jawbone or maxilla, which serve as a support for fixed dental prostheses. These prostheses can be made of materials such as porcelain or zirconia, which mimic the appearance of natural teeth.

Zirconia prosthesis for dental implants

The zirconia prosthesis is a white ceramic used to make the internal structure of a dental implant prosthesis. It is currently considered the most aesthetic material for this purpose, as its shade is very similar to natural dentin and it also allows more light to pass through (translucency).

Zirconia provides a metal-free crown with high strength and aesthetics

A crown with a zirconia structure is an aesthetic alternative to a metal structure (metal-free crown). Although very resistant, the metal structure can leave the crown looking darkened when exposed to light.

In addition, if the patient has remodeling of the gums over time, a grayish line can appear at the gingival level and cause aesthetic compromise in the smile. For this reason, metal frameworks are often recommended for posterior crowns (which require a lot of strength, but not much aesthetics).

The computer-designed zirconia prosthesis

Initially, the teeth are scanned using intraoral scanners to generate a 3D digital mold. The design of the crown is made on a computer and executed on 3D tooth printers.

Finally, the crown receives a layer of porcelain that makes it look very similar to a natural tooth and can be fixed onto the dental implant. Among the indications for zirconia crowns on implants are single crowns, bridges and complete fixed prostheses.

Planning is important for successful dental implant treatment

Dental implants are performed in the dentist’s office under local anesthesia, with care and attention from the implant dentist. Even those who are afraid of the dentist can have bone implants without any problem. Titanium implants are placed inside the bone and you have to wait three to six months for osseointeraction (calcification of the implant in the bone).

Once healing has taken place, the patient can wear a temporary prosthesis which allows them to better maintain their daily activities and also prevents the process of bone resorption (when the area is left without teeth for a long time, it can lose bone volume). Also during this period, the dentist makes the molds for the fixed dental prosthesis that will be placed after the implants have osseointegrated.

Temporary prosthesis for dental implants – planning

Despite the initial discomfort due to the implants and also the temporary prosthesis, the patient should wear it most of the time to facilitate adaptation. At first, the patient may feel nauseous, produce more saliva than usual and their pronunciation may also be strange. Of course, any discomfort in the meantime is temporary and must be overcome in order to achieve the best possible result.

However, in some cases, the dentist may opt to carry out the immediate loading procedure, in which the temporary prosthesis is fixed immediately after the implants are placed. This takes a maximum of 72 hours and, like all treatments, must be planned in advance to avoid problems in the future.

Porcelain veneers: a beautiful and attractive smile

To improve the appearance of the teeth and correct any damage they have suffered, patients have at their disposal porcelain veneers or dental contact lenses, which are thin sheets of porcelain, customized for the patient and bonded to the front of the front teeth. In the porcelain veneer technique, the dentist needs to:

  • Wear away a LARGER amount of the tooth enamel (approximately 1mm).
  • SILICONE molds and impression trays are made. While the permanent veneers are being prepared, the patient wears the temporary veneers.
  • The material used is generally common feldspathic porcelain, made using a handmade technique (more fragile).

The purpose of wearing down the teeth is to make room for the porcelain veneer and increase its thickness. Without wear, the teeth would be overcontoured, or have too much volume. In addition, the porcelain veneer might not fit so perfectly on the tooth.

In the contact lens technique, the dentist needs to:

  • Wear away a LESSER amount of the tooth enamel (approximately 0.3mm).
  • Molds are made with an INTRA ORAL SCANNER. While the permanent veneers are being prepared, the patient wears temporary lenses or a mockup. Sometimes it is not necessary to use temporary lenses because the wear does not affect the aesthetics.
  • The material used is generally porcelain MADE ON A COMPUTERIZED MILL (more resistant and precise).

Like any restoration, porcelain veneers need to be properly cared for and periodically reassessed. Patients should also maintain the hygiene of the tooth surfaces. Immediate loading of dental implants

Rapid (immediate) loading is possible thanks to scientific advances. Today it is possible, in selected cases, to enter the office without teeth and leave at the end of the day with all the teeth fixed on dental implants with immediate loading. This technology gives patients great satisfaction due to the rapid results.

Immediate rapid loading can be planned for individual dental implants, or even total dental implants, the so-called protocol prosthesis. Its feasibility should be assessed in consultation with the implant dentist.

When to load implants quickly:

Implants with this rapid loading technique can be used in the following cases:

  • For total implants (replacement of dentures with all teeth fixed)
  • Several teeth (usually teeth or tooth roots with periodontal disease, badly decayed teeth as well as old dentures)
  • For a single tooth (mainly in the anterior region)
  • Fixed prosthesis of some teeth (in selected cases)

The rapid loading technique can be indicated for a single tooth or for all teeth.

Indications for immediate rapid loading dental implants

The dental implant technique with immediate loading can be carried out when bone quantity and quality are normal and also when occlusal habits are favorable for the procedure to be carried out so as not to overload the site with chewing.

Therefore, the immediate loading technique depends on certain conditions of the patient’s dental arch and general oral health. The conditions, planning requirements and feasibility of the implant with immediate loading are assessed individually in consultation. Not all cases can be immediately loaded.

Therefore, the dental implant technique with immediate loading can be indicated for the following cases: people with a completely toothless lower arch, a full arch with a few teeth, single or individual teeth in the anterior region of the mouth, and in some cases fixed prostheses on a few teeth, depending on the region. In all cases and situations, the main course of action is to consult your dentist and carry out a full assessment to determine what and how will be done.

Only this assessment can determine whether an immediately loaded dental implant is indicated or whether another rehabilitation procedure would provide a better result. The technique is not always used in all cases, as it depends on the patient having some specific and favorable conditions that are assessed in consultation.

We must therefore plan your case carefully in order to achieve the expected success. The procedure aims to install a temporary prosthesis immediately after the implants have been placed. The time it takes to make the final prosthesis will be discussed with the dentist and varies from case to case.

Types of dental implants

If your concern is to complete your dental implant treatment as soon as possible, you can use fast-healing implants. The Swiss company Straumann is responsible for producing the most modern dental implants that allow rapid healing. Under ideal conditions, without bone grafting and good locking, these implants normally heal in between 1 and 2 months. So if your concern is time, opt for Straumann implants. These implants can be used for single implants or total implants.

In addition to titanium implants, there are also ceramic implants, an option for those who want a metal-free implant treatment.

Types of dentures

The type of dental crown or type of prosthesis to be used in your treatment should certainly also be evaluated, in order to shorten its execution time. If possible, opt for computerized crowns or prostheses. These modern prosthesis models use 3D tools that speed up the execution time and offer greater quality and naturalness.
